Most people only start thinking about the meningococcal vaccine when something forces the issue. A visa requirement. A university enrolment form. A friend who travelled to West Africa. The disease this vaccine protects against moves fast and hits hard. Even with treatment, a meaningful number of patients do not survive. Many who do are left with permanent damage. Once you understand that, the decision to vaccinate becomes much simpler.
What Is Meningococcal Disease and Why Does It Matter?
Meningococcal disease is a serious bacterial infection. It can infect the lining of the brain and spinal cord, or it can spread through the bloodstream. Both are medical emergencies.
What makes this disease different is the speed. A patient can go from feeling unwell to critically ill within hours. Even when treatment starts early, fatality rates remain significant. Survivors can be left with hearing loss, brain damage, or amputated limbs. This is not a condition where waiting to see how you feel tomorrow is a reasonable plan.
The bacteria spread through close contact: shared saliva, crowded living spaces, prolonged time near someone who carries it. This is why dormitories, pilgrimage gatherings, and communal housing are all flagged as higher-risk environments.
MenACWY vs MenB: Which Vaccine Do You Actually Need?
This is the question I get most often. The answer depends on where you are going and what you are doing.
MenACWY covers four strains of the bacteria in one shot. It is the vaccine required for Hajj and Umrah. It is what most US universities ask for. It is the standard travel vaccine for sub-Saharan Africa. For most healthy adults, one dose gives solid protection.
MenB covers a different strain entirely. Serogroup B is the most common strain in Thailand, and it is the dominant strain in Europe, Australia, and on university campuses in the UK and US. MenB requires two to three doses spaced weeks apart, so timing matters if you are preparing for a university departure.

Some people need both. Students heading to a UK university, or anyone with a weakened immune system travelling to a high-risk region, get much broader protection with both vaccines together. A short consultation will help you decide without guessing.
Who Should Get the Meningococcal Vaccine?
The patients I see most often fall into a few clear categories.
Travellers heading to high-risk regions. Sub-Saharan Africa, particularly the meningitis belt stretching across Central Africa, Saudi Arabia during pilgrimage season, and parts of South Asia all carry elevated risk. If you are flying from Bangkok to any of these destinations, this vaccine belongs on your list.
Students going abroad. Expat families sending children to universities in the US, UK, or Australia face real, immediate requirements. Many US universities will not allow dormitory check-in without proof of MenACWY. UK and Australian universities strongly recommend MenB for students in shared accommodation.
People with certain medical conditions. If you have had your spleen removed, have HIV, or have a condition affecting your immune system, your risk from meningococcal disease is significantly higher. In these cases, a two-dose primary series is usually recommended, with boosters every five years.
Long-term Bangkok residents who have never been vaccinated. Serogroup B does circulate here. If you live in communal housing or have no vaccination history for this disease, it is worth raising with a doctor.
When Is the Meningococcal Vaccine Compulsory?
There are two situations where you will not get past the gate without this vaccine.
The first is Saudi Arabia. All pilgrims travelling for Hajj or Umrah must show proof of MenACWY vaccination. The certificate must be dated within the last five years and administered at least ten days before arrival. This is a hard requirement. Bangkok has a significant Muslim community, and I see Hajj-bound patients every year who have left this too late. Book early. The ten-day window before arrival cannot be shortened.
The second situation is US university enrolment. Many American universities require proof of MenACWY for students living in dormitories. Some also require documentation for MenB. If the enrolment paperwork asks for a meningococcal vaccine record, your child needs the actual vaccine, not just a note from a doctor.
High-Risk Destinations from Bangkok
Bangkok sits at the centre of a lot of international travel. The meningitis belt in sub-Saharan Africa runs from Senegal in the west to Ethiopia in the east and sees seasonal outbreaks, particularly between December and June. Travel to Ghana, Nigeria, Mali, Niger, Ethiopia, or surrounding countries warrants MenACWY vaccination.
Saudi Arabia during pilgrimage season is in a category of its own. Millions of people from dozens of countries gather in close proximity, and vaccination is mandatory.
Extended travel to India, Nepal, or Pakistan, particularly in crowded areas, is another context where your doctor may recommend vaccination.
Dosing, Timing, and Boosters
For most healthy adults, MenACWY is a single dose. Protection builds over about two weeks, so get vaccinated at least two weeks before departure. The Saudi Arabia certificate requirement also demands a ten-day minimum gap, so earlier is always better.
If you are immunocompromised or have had your spleen removed, your doctor may recommend two doses given one to two months apart, then a booster every five years.
MenB follows a different schedule. Bexsero requires two doses at least one month apart. Trumenba requires two or three doses depending on the schedule chosen. Because MenB protection can wane within one to two years, people in ongoing high-risk situations may need to revisit this sooner than they expect.
One thing I see repeatedly: patients who were vaccinated five or more years ago and assume they are still covered. If that is you, and you are now planning travel to a high-risk region or preparing for Hajj, check whether you are still within the protection window. A booster may be due.
Side Effects and Safety
The meningococcal vaccine is well-tolerated. The most common reactions are a sore arm, mild fatigue, and sometimes a low-grade fever for a day or two. These are normal signs that your immune system is responding.
Serious reactions are rare. If you have previously had anaphylaxis after a meningococcal vaccine, tell your doctor before receiving another dose.
MenB can produce slightly stronger local reactions than MenACWY, particularly soreness at the injection site and flu-like symptoms for 24 to 48 hours. This is expected and does not mean anything is wrong.
