Medications for Type 2 Diabetes Available in Bangkok

If you are an expat living in Bangkok or a visitor who has just run low on diabetes medication, you are not alone. I see this regularly at my clinic. People assume they can hand a home-country prescription to a Thai pharmacy and walk out with their usual tablets. It does not work that way.

The good news is that Bangkok has excellent access to modern diabetes medication, from basic metformin to the newest injectable therapies. What you need is a short consultation with a licensed doctor here, and in most cases you can leave with a prescription the same day.

What Diabetes Medications Are Available in Bangkok

Bangkok’s private clinics and hospitals stock a full range of type 2 diabetes medication. Most of what your doctor at home prescribed is available here, sometimes under a different brand name but the same active ingredient.

The main drug classes available in Bangkok include metformin, sulfonylureas, DPP-4 inhibitors, SGLT2 inhibitors, GLP-1 receptor agonists, and insulin. Newer agents like semaglutide and tirzepatide are now available following Thai FDA approval.

Metformin — Still the Starting Point

Metformin is almost always the first medication prescribed for type 2 diabetes, and that is true in Bangkok just as it is in the UK, US, or Australia. It has decades of safety data behind it and is widely stocked across the city.

Most patients tolerate it well. Some get stomach upset in the first few weeks, especially on higher doses, and taking it with food usually helps. If your kidneys are not working well, metformin may not be suitable, which is one reason a blood test before prescribing matters.

Sulfonylureas and DPP-4 Inhibitors — The Middle-Ground Options

Sulfonylureas like glipizide and glibenclamide are inexpensive and widely available in Bangkok. The main risk is low blood sugar, especially if you skip meals, so these are used carefully and with clear guidance on what to watch for.

DPP-4 inhibitors, including sitagliptin and vildagliptin, are a gentler option with a low risk of hypoglycaemia. They are often added when metformin alone is not bringing HbA1c, your three-month average blood sugar, down to target.

SGLT2 Inhibitors — Blood Sugar Control With Added Benefits

SGLT2 inhibitors, including empagliflozin and dapagliflozin, cause the kidneys to pass excess glucose out in urine. Beyond blood sugar control, there is good evidence they protect the heart and kidneys in people with established cardiovascular disease or chronic kidney disease.

These medications are well-stocked in Bangkok’s private clinics and hospitals. A kidney function check is needed before prescribing them. Side effects can include urinary tract infections and genital yeast infections in some patients.

GLP-1 Receptor Agonists — Ozempic, Rybelsus, and Mounjaro in Bangkok

This is the class I get asked about most right now. Semaglutide, sold as Ozempic (weekly injection) and Rybelsus (daily tablet), is Thai FDA-approved and available in Bangkok through licensed private clinics. Tirzepatide, sold as Mounjaro, is also available and tends to produce stronger blood sugar reduction and weight loss than semaglutide alone, based on current trial evidence.

These are not over-the-counter medications. They require a clinical assessment, particularly if you have a history of heart disease, pancreatitis, or thyroid conditions. What About Shortages?

Ozempic supply has been inconsistent globally due to high demand. Bangkok’s situation improves with private clinic access, but it is worth asking at your appointment whether stock is currently available. Rybelsus, the oral tablet form of semaglutide, is often more reliably stocked.

Insulin Therapy in Bangkok

If you are already on insulin at home, you can continue it in Bangkok. Both rapid-acting and long-acting formulations are available at private clinics and major pharmacies. You will need a Thai doctor to assess your regimen and prescribe locally, especially if doses need adjusting based on changes in your diet or activity level.

Managing Diabetes When You Are Eating Thai Food

This is something I think every newly arrived expat with diabetes should hear. Thai food is genuinely difficult for blood sugar control. Jasmine rice is high-GI. Many sauces contain significant sugar. Even savoury dishes like pad see ew often arrive sweetened, and sugary drinks are everywhere, including plain-looking juices and teas.

I see patients whose blood sugar was well-managed at home but becomes erratic within weeks of arriving in Bangkok. It is not that their medication stopped working. The diet changed significantly. If this sounds familiar, a medication review makes sense.

Running Out of Diabetes Medication in Bangkok

If you have run out of medication, do not try to manage without it while waiting to go home. Uncontrolled blood sugar over days or weeks causes real harm.

If you have heart disease or kidney involvement, that changes which medication class is preferred. The consultation is a proper clinical review, not just a repeat of what you were on at home.

What I recommend for expats staying longer than a few months is to establish care with a local doctor early, not only when things go wrong. Regular HbA1c checks every three to six months, blood pressure and kidney function monitoring, and annual eye and foot checks are all part of good diabetes management.
