Ozempic (semaglutide) works on one gut-hormone pathway; Mounjaro (tirzepatide) works on two, and tends to show greater average weight loss in trials. Neither is universally best. The right choice depends on your health, tolerance, other conditions and availability, which is why a doctor should decide with you.
This is the question I am asked more than almost any other: should I choose Ozempic or Mounjaro? It is a fair question, because both are effective and both are widely talked about among expats in Bangkok. The honest answer is that they are more similar than the marketing suggests, and the better one is the one that fits you.
Below I compare them the way I would in a consultation: mechanism, results, side effects, and the practical factors that actually tip the decision. This is educational, not a substitute for a personal assessment.
The core difference: one hormone vs two
Ozempic contains semaglutide, which acts on the GLP-1 pathway. Mounjaro contains tirzepatide, which acts on both GLP-1 and GIP. Both reduce appetite and improve blood-sugar control; the dual action of tirzepatide is the main reason it often produces larger average weight loss in studies. For the mechanism in depth, see our guide to semaglutide injections.
Results: what the trials suggest
Head-to-head and separate trials generally show tirzepatide achieving greater average weight loss than semaglutide. That is a real difference at the population level, but it is an average. Some people do exceptionally well on semaglutide, and tolerance varies. We do not guarantee any specific result with either medicine.
Side effects: broadly similar
Both medicines share a similar side-effect profile, mostly digestive, such as nausea, reduced appetite and altered bowel habits, easing as the body adjusts. Both carry the same categories of contraindication. Our detailed guide to GLP-1 side effects applies to both.
Practical factors that tip the decision
Availability
Global demand means either brand can be in short supply at times. Sometimes the practical choice is what is reliably obtainable. See our guides to Ozempic in Thailand and Mounjaro in Thailand.
Your health profile and tolerance
Existing conditions, other medications and how you tolerate early doses all matter. Eligibility is covered in are you eligible for weight-loss injections?
Cost
Pricing differs by dose and brand and is confirmed at consultation. See cost in Bangkok.
When a doctor should make the call
Because both medicines affect blood sugar, digestion and hydration, and because some people have conditions that make one option safer than the other, this decision belongs in a consultation. If you develop severe abdominal pain, persistent vomiting or signs of an allergic reaction on either drug, seek prompt care. Our 24/7 clinic can review you the same day.
Not sure which is right for you? A screening consultation is the best way to decide. Start with our overview of weight-loss injections in Bangkok or contact the clinic.
Frequently asked questions
Which loses more weight, Ozempic or Mounjaro?
On average, tirzepatide (Mounjaro) shows greater weight loss in trials, but individual results vary and averages are not guarantees.
Are the side effects different?
They are broadly similar, mostly digestive and usually easing over time. Individual tolerance can differ between the two.
Can I try one and switch if it does not suit me?
Switching is sometimes possible under medical guidance, so dosing and side effects are managed safely.
Is one cheaper than the other?
Cost depends on dose, brand and duration and is confirmed at consultation. Neither is reliably cheaper in every case.
How do I decide?
The best way is a consultation where a doctor weighs your health, tolerance, goals and current availability with you.
