This is one of the most common things I help expats with at our clinic.
The good news: it is straightforward. The examination is quick, usually under 30 minutes. You do not need to fast. You do not need a referral. In most cases, you walk out the same day with everything signed and stamped.
What Does the Work Permit Medical Certificate Check For?
The Thai Ministry of Labour requires that work permit applicants are screened for six conditions: leprosy, tuberculosis, drug addiction, chronic alcoholism, elephantiasis, and third-stage syphilis.
For almost every expat I see, the examination comes back clean. The syphilis screen is a simple finger-prick blood test, and results are usually ready within minutes at a clinic equipped to run it on-site. The rest of the assessment is a physical check: the doctor looks, asks a few questions, and documents findings.
This is not a deep health screening. It is a targeted check for the specific conditions the Ministry of Labour requires. If you are generally healthy, it takes about 15 minutes.
Medical Certificate for a Thai Driver Licence
The Department of Land Transport has its own requirements, and they differ slightly from the work permit process. The certificate needs to confirm that you are not colour blind and that you do not have conditions that would make driving unsafe.
The examination includes a basic vision check, a colour blindness test, and a brief physical assessment. Most patients are in and out in under 20 minutes.
One thing worth knowing: some Department of Land Transport offices ask for two separate certificates if you are applying for both a car licence and a motorcycle licence at the same time. Tell the doctor at the start of your appointment if that applies to you. We can issue both on the same visit.
How Long Is a Medical Certificate Valid in Thailand?
This is where I see a lot of expats get caught out, especially people who book their examination weeks before they actually need to submit their paperwork.
For work permits, the certificate must be dated no more than 30 days before you file your application with the Ministry of Labour. This applies to both new applications and annual renewals. If your certificate is older than 30 days on the day you submit, you will need a new one.
For driver licences, the Department of Land Transport generally accepts a certificate that is current at the time of application, though guidance can vary by office. Book your appointment close to the date you plan to submit. Do not get it done a month early and assume it will still be accepted.

Work permits and driver licences are the two most common requests, but they are far from the only ones.
Retirement Visa and Long-Stay Visa
Some visa categories, including the retirement visa and certain long-stay types, require a medical certificate as part of the application. Requirements can vary depending on the visa type and the issuing embassy or consulate. Fit-to-Fly Certificate
If you have had a recent illness, surgery, or hospitalisation, your airline may ask for a fit-to-fly certificate before you board. This is a physician’s written confirmation that you are medically safe to travel by air. It comes up most often with pregnancy, post-operative recovery, and acute illness. Some employers, particularly international companies and schools, ask for a pre-employment health letter before your first day. What If Your Certificate Is Rejected?
It happens occasionally, and usually for one of two reasons. Either the certificate was issued by a clinic not recognised by the relevant authority, or the document format does not match what was expected.
If a government office or employer rejects your certificate, ask them in writing what is missing. Bring that information back to us. In most cases, the fix is a reissued document in the correct format, not a repeat examination.
