First-time users should start at the lower end. Jumping to 20mg when 10mg would have worked just increases the chance of side effects: headache, flushing, back pain, or nasal congestion. These are common and usually mild, and they often ease with repeated use or a lower dose.
Do not adjust your dose based on what worked for a friend. Body weight, age, kidney function, and other medications all affect how tadalafil behaves. A doctor works this out with you quickly.
What Else Can Tadalafil Treat Beyond Erectile Dysfunction?
Most patients do not know this. Tadalafil is also approved for BPH. If you are over 45 and getting up at night to urinate, feeling like your bladder never fully empties, or dealing with a weak flow, tadalafil at 5mg daily can genuinely help. It relaxes the smooth muscle in the prostate and the bladder neck.
This makes daily 5mg a particularly useful option for older male expats who have both ED and urinary symptoms. One medication, two problems addressed.
Tadalafil is also used in specialist settings for pulmonary arterial hypertension. That is outside the scope of a general consultation, but it is worth knowing the drug has a broader clinical profile than most people expect.
Who Should Not Take Cialis: Drug Interactions and Contraindications
This section matters more than most people realise, especially for expats already on medication from home.
Nitrates are an absolute contraindication. If you take any nitrate medication, including nitroglycerin for chest pain or isosorbide, you must not take tadalafil. The combination can drop blood pressure severely and quickly, and there are no exceptions.
Alpha-blockers, often prescribed for high blood pressure or BPH, can also lower blood pressure when combined with tadalafil. This is not always a contraindication, but it requires a medical review. Many men on tamsulosin or doxazosin can still take tadalafil safely, but a doctor needs to make that call.
Grapefruit and grapefruit juice can raise tadalafil levels in your blood. It is not dangerous in small amounts for most people, but worth knowing. Some HIV antivirals have the same interaction.
