It is one of the most recognised erectile dysfunction treatments in the country, largely because it is a locally produced generic and therefore far cheaper than branded Viagra. Patients often ask me whether it is the real thing, whether it is safe, and how to take it. This article answers those questions honestly, from a doctor’s point of view.

What is Sidegra?
Sidegra is a brand of generic sildenafil manufactured in Thailand by the Government Pharmaceutical Organization. Sildenafil is the same active ingredient found in Viagra. When the patent on branded Viagra allowed generics, Sidegra became a popular, affordable alternative. Because it is a generic, the medicine inside is chemically the same as the original; the differences are in the brand name, appearance, and price rather than the underlying drug.
How Sidegra works
Sildenafil belongs to a group of medicines called PDE5 inhibitors. In simple terms, it relaxes the blood vessels supplying the penis so that, when you are sexually aroused, more blood can flow in and produce a firmer erection. An important point that surprises some men: it does not create desire or work on its own. You still need to feel aroused for it to do anything. It typically needs to be taken a little while before sex, and food and alcohol can affect how quickly it works.

Dosage: why your doctor decides
Sildenafil comes in more than one strength, and the appropriate dose varies from person to person depending on your age, other medicines, kidney and liver function, and how you respond. For that reason I will not print a dose here: your doctor will advise the right starting dose for you and adjust it if needed. Taking more than advised does not make it work better and increases the risk of side effects. Never combine doses or double up because one tablet seemed slow to work.
Safety and side effects
For most healthy men, sildenafil is well tolerated. Common, usually mild side effects include headache, facial flushing, a stuffy nose, indigestion, and temporary changes in vision such as a blue tinge. These generally pass as the medicine wears off.
The serious safety issue is drug interactions. Sildenafil must never be combined with nitrate medicines used for chest pain, as together they can cause a dangerous drop in blood pressure. It also needs caution with certain blood pressure treatments and with some medicines used for prostate symptoms. This is precisely why a doctor should review your medicines and health before you start.
The risk of buying from unregulated sources
Tablets sold cheaply on the street or from unverified online sellers may be counterfeit, wrongly dosed, or contaminated. Buying through a proper clinic or pharmacy after a consultation means you know what you are actually taking.
When to see a doctor
See a doctor before starting Sidegra if you have heart disease, have had a stroke, take nitrates or multiple blood pressure medicines, or are unsure whether it is safe for you. Seek urgent care if you experience chest pain during sex, a sudden loss of vision or hearing, or an erection lasting more than four hours, which is a medical emergency. And if erection problems persist despite treatment, it is worth investigating the underlying cause rather than simply increasing tablets.
