Testosterone Therapy (TRT) in Bangkok: Testing, Treatment, and Men’s Health

Clinically reviewed by Dr. Ponlawat Pitsuwan, Physician, Doctor Bangkok. Last reviewed: July 2026

Testosterone therapy (TRT) replaces the hormone in men who have both low blood levels and genuine symptoms. In Bangkok, it starts with a morning blood test, a clinical review, and careful monitoring. It is a long-term medical treatment, not a quick fix, and it is not right for every man.

As a physician working with expats and travellers in Sukhumvit, I see a steady stream of men in their 30s, 40s and 50s who feel flat, tired, and not quite themselves. Many have read about testosterone online and want to know whether it is the answer. Sometimes it is. Often the picture is more complicated, and the honest job of a good clinic is to test properly, explain the trade-offs, and only treat when the evidence supports it.

This guide walks through what low testosterone actually is, how we test for it, what treatment and monitoring involve, the real risks, and roughly what it costs here in Bangkok. My aim is to give you the same information I would give a patient sitting across from me, so you can decide whether getting checked is worth your time.

What low testosterone actually is

Testosterone is the main male sex hormone. It supports libido, erections, muscle and bone strength, red blood cell production, mood and energy. Levels peak in early adulthood and drift down slowly with age, typically around one percent a year from your late 30s. That gradual decline is normal and does not, on its own, mean you need treatment.

The medical condition we treat is called hypogonadism, sometimes shortened to “low T”. It means the testosterone level is genuinely below the normal reference range AND you have symptoms that fit. Both halves matter. A low number with no symptoms, or classic symptoms with a normal number, usually points somewhere other than testosterone. To understand the numbers themselves, see our guide to normal testosterone levels by age.

Symptoms that make me think about testosterone

No single symptom proves low testosterone, because most of them overlap with stress, poor sleep, alcohol, thyroid problems and depression. But when several appear together, it is worth a blood test.

The more specific signs

Reduced sex drive, weaker or less frequent erections, fewer spontaneous morning erections, and shrinking testicle size are the symptoms most closely tied to low testosterone. These carry more weight than the general ones.

The less specific signs

Fatigue, low mood, poor concentration, loss of muscle, more belly fat and disturbed sleep are common but easily explained by other things. In Bangkok, heat, long hours, jet lag and heavy nights out do a lot of this on their own. Our detailed look at low testosterone symptoms and causes covers this in full.

How testing works at our clinic

Testosterone is measured with a simple blood test, but timing matters. Levels are highest in the morning, so we ask you to come in before 11am, ideally fasting. Because a single result can be misleading, low readings are confirmed with a second test on another day.

A proper work-up looks beyond one number. We usually check total and free testosterone, and hormones such as LH, FSH, SHBG and prolactin to work out where the problem sits. We also screen for the common mimics: thyroid function, blood count, blood sugar and sometimes iron studies. You can read exactly what is included in our guide to the testosterone test in Bangkok. Our on-site lab means most results come back the same day or the next morning.

TRT options and what monitoring involves

If testing confirms genuine deficiency, treatment replaces testosterone to bring you back into a healthy range. The main forms are injections given every one to a few weeks, and daily topical gels applied to the skin. Each has trade-offs in convenience, cost and how steady your levels stay. Your doctor will advise which suits your situation and lifestyle.

TRT is not a set-and-forget prescription. We recheck your testosterone, blood count (haematocrit) and PSA at intervals, starting a few months in and then periodically. Monitoring is how we keep treatment both effective and safe, and it is a permanent part of being on therapy rather than an optional extra.

Safety, risks and honest expectations

When it is genuinely needed and properly monitored, TRT is a well-established treatment. But it carries real considerations. It can thicken the blood, which is why we track haematocrit. It usually reduces fertility and sperm production, so it is generally not the right choice for men still hoping to father children. It can worsen untreated sleep apnoea and needs care in men with certain heart or prostate conditions.

I am also honest about what TRT will not do. It is not a shortcut to bodybuilding, not a general anti-ageing tonic, and not a fix for tiredness caused by poor sleep or stress. Anyone who promises those things is overselling. Some men with low testosterone also have erectile difficulties that need their own assessment; see our guide to erectile dysfunction treatment in Bangkok.

Who is, and is not, a candidate

Good candidates are men with consistently low blood levels on repeat testing plus matching symptoms, who understand the monitoring and are not currently trying to conceive. Men whose numbers are normal, whose symptoms are better explained by lifestyle or another condition, or who want treatment purely for performance are usually better served by addressing the real cause first. A general health check-up in Bangkok often uncovers the more likely explanation.

What it costs and how to start

Cost depends on the tests you need and, if you proceed, the type of treatment and monitoring. Rather than quote figures that may not fit your case, we price everything transparently at consultation so you know what you are paying for before anything begins. Our breakdown of TRT cost in Bangkok explains the moving parts. To start, book a consultation and morning blood test; from there your doctor will interpret the results with you and advise on next steps.

One related question I am often asked is about hair. Testosterone is converted into DHT, the hormone most linked to male pattern baldness, which is why some men worry about TRT and hair loss. Our article on DHT and hair loss explains the connection.

When to see a doctor

See a doctor if you have several of the symptoms above together, particularly reduced libido or erection problems, or if low mood and fatigue are affecting your daily life. Do not buy testosterone online or self-inject; unmonitored use is where the real harm happens. Seek urgent care for chest pain, severe breathlessness, or signs of a blood clot such as a hot, swollen, painful leg.

Frequently asked questions

Do I need TRT if my testosterone is a bit low?

Not necessarily. Treatment is considered when levels are genuinely low on repeat testing and you have matching symptoms. A slightly low number with no symptoms usually does not need testosterone, and lifestyle factors are often worth addressing first.

Is TRT a lifelong treatment?

In most cases of age-related or permanent deficiency, yes, testosterone works while you take it and levels fall again if you stop. Your doctor will discuss the commitment, including ongoing monitoring, before you start.

Will TRT affect my fertility?

Usually yes. TRT tends to reduce sperm production and can impair fertility, so it is generally not recommended if you are trying to conceive. Tell your doctor if fatherhood is a goal, as there are alternative approaches.

How soon would I feel a difference?

Effects build gradually. Libido and mood often improve within weeks, while changes in energy, body composition and strength can take several months. Your doctor will set realistic expectations and review your progress.

Can I just get a testosterone test as a walk-in?

Yes. We are open 24/7 and you can walk in for a morning blood test. For the most accurate result, come before 11am and ideally fasting, and a doctor will go through the findings with you.
