Types of medical certificate we issue in Bangkok

The format of the certificate depends on what it is for. A sick leave certificate confirms you were medically unfit for work on specific dates and is usually one page. A work permit certificate in Thailand uses the official Ministry of Labour template and declares you are free from leprosy, tuberculosis, drug addiction, chronic alcoholism, third-stage syphilis and elephantiasis, which is why it requires a specific physician examination. A visa medical follows whichever format the destination country or embassy requires, which may include additional blood work and chest X-ray. A driving licence certificate in Thailand uses the Department of Land Transport format and confirms colour vision, general fitness and no disqualifying conditions.

School and sports certificates usually confirm fitness to participate in physical activity or return after an illness or injury. Fit-to-fly letters certify that you are medically stable enough to board a commercial flight, and airlines will often specify the wording they need (for example post-pregnancy, post-surgery, or recent illness). Insurance forms vary hugely, from a short claim note to a detailed medical report, and we charge based on the complexity of the form.

What to bring to your appointment

To issue a medical certificate in Bangkok quickly and correctly, bring a valid passport or Thai national ID, any blank forms the employer, embassy, school or insurer has given you, and a list of current medications. For a work permit certificate, bring your old work permit and, if this is a renewal, your previous certificate. For a visa medical, know exactly which country and visa category you are applying for, because requirements differ. For a fit-to-fly letter, bring the airline’s form or tell us the carrier and the flight details, because airlines have different cut-offs (for example pregnancy over 36 weeks). If the certificate is for a return to work after an accident or surgery, bring any discharge summary or imaging you have.

When a medical certificate is not enough and you need a proper assessment

See a doctor if: you are requesting a sick leave certificate but are still actively unwell with fever, ongoing vomiting, severe pain, breathing difficulty, confusion, chest pain or any red-flag symptom. The certificate can still be issued, but the priority is to treat the underlying condition first. A certificate without treatment is a wasted visit. Please tell reception you are unwell so we book you as a clinical consultation, not just a paperwork slot.

A medical certificate is also not a way to bypass a real assessment. If you are unfit for work, we need to document why. If you are unfit to fly, we need to know the reason so the airline can accept the letter. If the certificate is for a work permit and we find something clinically significant during the examination, we will discuss it with you and arrange further testing rather than ignore it. This is the difference between a physician-issued certificate and a stamp from a walk-in shop: the document is only useful if it is honest.

Prevention and early detection: the visit is also a chance to screen

Prevention point: A medical certificate appointment is often the only time a healthy adult sees a doctor all year. Use it. If you are in the clinic for a work permit or visa medical, take the opportunity to check your blood pressure, update your vaccinations, and discuss anything that has been on your mind. It costs very little to add a basic wellness screen to the same visit and can pick up hypertension, diabetes or high cholesterol before they become a problem.

For adults over 35, we recommend combining the annual work permit medical with a basic health check up in Bangkok every year, and a full screen every 2 years. Travellers collecting a visa medical should also review their vaccinations at the same time, particularly tetanus (Tdap every 10 years), hepatitis A and B, typhoid for longer trips, and rabies pre-exposure if travelling to rural areas. A single 30-minute visit can cover the certificate, the vaccinations, and the screening conversation.
